Instructions and Screenshots
1. Select 'Create Pathway', fill in the pathway information and click 'Next'.
2. Select 'Create my own' pathway and click 'Create'.
3. Click '+Add Stage' and select the relevant stage.
4. Click 'Create New Task', fill in the task information and click 'Next'.
5. Click 'Create New Form' and click 'Next'.
6. Click 'Add Data Field' and fill in the form name.
7. When filing in the Data Type select 'Dropdown'. This will give you the option of filling in a number of stages.
8. The stages of the form can be made mandatory by switching on the 'Mandatory' node on the right hand-side.
9. Fill in the 'Form Name' and click 'Create' to make the form.

This video shows how to set up a task with a check box requirement. The Task is to set up a drop-down box that allows the Team Member to indicate which drop-down choices should be available and to set up the task to enable selection by the Team member assigned to the Task
